Form	Description
Booklet - Information for Unit Leaders	This booklet explains the purpose, organization and operation of the Order of the Arrow for a unit leader who may not be familiar with the OA.
Form - Lodge - Unit Election Request	This form is used by a unit to request to have a unit election conducted for the troop. It should normally be submitted by the SPL acting on behalf of the PLC.
Form - OA Troop Representative Registration	This form is used for the unit to register their OA Troop Representative with the Lodge and Council.
Form - Lodge - Adult Nomination 08	This form is used, in conjunction with a unit election, for a unit to nominate a registered adult leader for membership in the OA.
Form - Lodge - Presentation Request	This form is used for a unit to request a presentation explaining the order of the Arrow and the benefits, to the unit and to the individual Scout, of membership in the OA.
Form - Lodge - Service Request	This form is used by a district committee and/or unit to request that the OA perform certain services in conjunction with district events or unit ceremonies. These services are explained on the form.
Guide - Lodge - Election Procedure for Unit Leaders	This guide provides the unit leader with information they need to have, on the conduct of a unit election and what the involvement is of the various unit leaders is, prior to a scheduled unit election.
